STATE OF CALIFORNIA — License Renewal Forms

Use our interactive forms to renew your license. You will need the Adobe Acrobat Reader to use and print these forms. Use this link if you need to download the Adobe Acrobat Reader.

When you have completed the form, print it out, sign it, and mail it with your check to the DRE.
